Description

Conduct patient evaluations in both inpatient and outpatient environments, crafting tailored treatment plans to meet individual needs
Implement physical therapy techniques that enhance mobility, build strength, improve balance, and restore function
Work collaboratively with a diverse team of nurses, doctors, case managers, and rehabilitation professionals to maximize patient outcomes
Provide education to patients and their families regarding therapeutic objectives, discharge processes, and at-home exercises
Maintain accurate documentation of evaluations, treatments, and patient progress following facility and regulatory standards
Engage in interdisciplinary cooperation by participating in care team rounds and conferences to ensure seamless care delivery
Education
~ Doctor of Physical Therapy (DPT) or an equivalent degree from a recognized program

Certifications
State licensure as a Physical Therapist
CPR certification is preferred
Add-on certifications (neurological, geriatric, etc.) are beneficial but not mandatory
Skills
Previous experience in a clinical setting is highly preferred
Competence in treating patients with complex medical and mobility challenges
Excellent teamwork and communication skills
Familiarity with digital charting and clinical documentation systems
