About the Role Transform bedside care with AI. As Circadia’s on-site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions—reducing preventable readmissions. You’ll blend direct patient care with tech-to-bedside integration through hands-on training, live demos, and shadowed assessments.

This is a full-time, embedded NP role requiring Monday–Friday, on-site presence at our partner facility in Clovis, CA.

Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; coordinate with facility providers.

Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.

Data into action : Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.

30+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts.

Quality & product feedback : Surface field insights to improve workflows and tech integration in a clinical setting

Visits: Target 30 patient visits/day

Adoption & impact: Demonstrated workflow adherence at facilities; Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.

• 2+ years NP experience with complex adult populations; comfortable traveling locally.
• English required

Google Suite, multiple EMRs, Slack, AI transcription/tools.

Spanish is preferred.

As a full-time Nurse Practitioner, you will be employed by Circadia Health, Inc. The anticipated annual base salary range for this full-time position is $130,000 - $160,000 . Annual salary is only one part of an employee's total compensation package at Circadia Health. 100% company-paid medical, dental, and vision coverage
