Registered Nurse (RN) - Home Health Care - Nights

Role Overview Seeking a Registered Nurse (RN) to join the Transition team for full-time night shifts, providing skilled nursing care in patients' homes during a provisional period of 1-90 days. The team focuses on efficiently transitioning patients to home care while recruiting and training new full-time caregivers. Nurses assess, plan, implement, and evaluate patient care, monitor physiological and psychosocial changes, and document nursing diagnoses in accordance with prescribed orders.
Responsibilities Utilize the nursing process for patient assessment and care delivery
Provide skilled nursing services during home assignments
Assess changes in patient condition and document findings
Collaborate with healthcare team members to ensure quality care
Qualifications & Skills Licensed as an LPN/LVN in the practicing state
Current TB or Chest X-Ray and BLS certification
One year of nursing experience, with tracheostomy and ventilator experience preferred
