Description

Conduct evaluations for patients in inpatient and outpatient environments, crafting personalized treatment plans
Implement effective physical therapy interventions aimed at improving mobility, strength, balance, and overall function
Work in tandem with nurses, physicians, case managers, and other rehabilitation professionals to optimize outcomes
Guide patients and their families through understanding therapeutic objectives, discharge procedures, and home exercise regimens
Maintain thorough documentation of evaluations, treatments, and patient progress in line with facility and regulatory standards
Engage in collaborative care team rounds and interdisciplinary conferences to ensure seamless patient care
Education
~ Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications
Current state licensure as a Physical Therapist
CPR certification is preferred
Any specialized certifications (e.g., in neurological or geriatric care) are a valued addition

Prior experience in a similar role is highly desirable
