• Recent experience conducting comprehensive assessments of patients.
• Experience in a home care setting, working with Certified Home Health Agencies, Licensed Home Care Service Agencies, or Managed Long-Term Care.
• Experience providing primary care to homebound individuals and care management for persons receiving home and community-based services.

Description:
The role requires the ability to review health status questions and make evaluations based on UAS criteria for enrollment.
Candidates must be comfortable working with technology in the workplace.
Knowledge of the principles and practices of professional nursing is essential.
Understanding state and agency regulations governing nursing practices is necessary.
Strong organizational and communication skills are required, including the ability to communicate effectively with a diverse population.
Candidates should possess sensitivity to the concerns of others.
The ability to work independently in a fast-paced, deadline-oriented environment is crucial.
The job includes performing initial in-home evaluations of patients.
Responsibilities involve reviewing consumer medical documentation relevant to each case.

Candidates will discuss healthcare options and supports needed with consumers.
Initiating communication for enrollment into managed care plans is part of the role.
