Description
Assess patients in either inpatient or outpatient settings and create tailored treatment plans
Administer interventions in physical therapy targeting mobility, strength, balance, and overall function
Collaborate closely with nurses, physicians, case managers, and multidisciplinary team members to achieve optimal patient outcomes
Educate both patients and their families on therapeutic objectives, discharge planning, and home exercises
Record evaluations, treatments, and patient progress in alignment with facility and regulatory standards
Engage in care team rounds and interdisciplinary meetings to maintain quality care delivery
Education
~ Doctorate in Physical Therapy (DPT) or related degree from a recognized program

Certifications
State licensure as a Physical Therapist
CPR certification is preferred
Additional certifications (e.g., neurological, geriatric) are advantageous but not mandatory
Skills
Prior professional experience is highly valued
Ability to manage patients with complex medical and mobility challenges
