Sapphire Nursing at Meadow Hill , a skilled nursing facility located in Newburgh , New York is actively looking for a caring, dedicated FULL TIME Physical Therapist to provide quality care to its residents. The Physical Therapist evaluates and treats patients/residents, communicates with their families, physicians, and other health team members, and maintains all necessary documentation of services in the patient’s medical records. Physical therapy services include: treatment planning & implementation, discharge planning, testing patients’ physical abilities, analyzing rehabilitation goals, and consulting with the rehab team and other health team members to coordinate the most effective treatment program.
PT Duties Include:
Supervise the activities of physical therapy assistants.
Evaluate Residents conditions & develop appropriate physical rehabilitation plans.
Help Residents improve their mobility.
Relieve Resident’s pain using exercises, hands-on therapy & stretching.
Teach Residents how to use physical therapy equipment.
Help Residents through their tasks and monitor their performance.
Maintain Resident confidence & protects the facility by keeping information confidential.
Complies with federal, state, and local legal and professional requirements
Submit all billing logs and necessary information on a timely basis.
PT Requirements Include:
Graduate of an accredited Physical Therapy Program
Current New York State PT License
Nursing Home Experience preferred
