Key Responsibilities\n

• Conduct thorough bio-psychosocial evaluations to identify patients' underlying issues and barriers to treatment, enhancing their health status through tailored interventions.

• Collaborate closely with medical and support staff to develop comprehensive care plans that prioritize patient needs and promote treatment compliance.

• Utilize evidence-based therapeutic modalities, both individually and in group settings, to provide compassionate and effective care tailored to each patient’s unique circumstances.

• Document all social work activities meticulously in patients' medical records, adhering to departmental standards regarding content and quality.

• Provide education to patients and their families, helping them understand and internalize medical recommendations, fostering better adherence to treatment plans.

• Actively assess advocacy needs for patients and families and coordinate relevant community resources, emphasizing empowerment and self-determination.

• Stay informed about local community resources and services, integrating this knowledge into patient care plans as necessary.

• Engage in ongoing professional development through training and education, ensuring you remain current with the latest practices in social work.

• Participate in weekly team meetings to discuss case progress, sharing insights and strategies that enhance care delivery.

Patient Population / Client Focus\n

In this role, you will serve a diverse client base, primarily focusing on individuals experiencing mental health challenges associated with various life stressors, including trauma, depression, anxiety, and relational issues. You will apply your clinical expertise to reach underserved communities, addressing distinctive cultural and socioeconomic factors that influence mental health.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution.

• A current License as a Clinical Social Worker recognized by the State Board of Social Work Licensure.

Compensation and Benefits\n

Refreshed Mind Clinic offers a competitive salary range of $85,000 to $100,000 per year, commensurate with experience and qualifications.
