Key Responsibilities\n

• Conduct thorough biopsychosocial assessments tailored to the individual needs of clients, identifying mental health, substance abuse, and medical concerns.

• Provide a variety of behavioral health services, including individual and group counseling, crisis intervention, and treatment planning for diverse age groups.

• Facilitate group therapy sessions, utilizing your skills to engage participants and create a safe space for open discussion.

• Maintain accurate and up-to-date electronic documentation of treatment progress, including mental health evaluations, progress notes, and suicide risk assessments.

• Collaborate with primary care providers and community organizations to coordinate referrals and ensure comprehensive care for clients.

• Act as a liaison for clients, connecting them with local resources and community services, including medication assistance programs.

• Work with Quality Improvement teams to enhance service delivery and implement data collection strategies to better inform practice.

Patient Population / Client Focus\n

In this role, you will work primarily with a diverse clientele consisting of individuals, couples, and families from various cultural backgrounds, each with unique mental health needs. Your expertise will be crucial in understanding and addressing the social determinants of health that affect their mental well-being.

Qualifications\n

• A valid Licensed Clinical Social Worker (LCSW) license in the state of New York is required.

• A Master’s degree in Social Work (MSW) along with three (3) years of post-licensure clinical experience is essential.

• Strong knowledge of counseling techniques, crisis intervention strategies, and the ability to make informed, independent judgments regarding client care.

Compensation and Benefits\n
