Key Responsibilities\n

As a Licensed Clinical Social Worker, your daily responsibilities will include:

• Engaging with patients and their care partners via video calls and other digital communication methods to assess psychosocial needs in preparation for safe discharges and ongoing care management.

• Formulating individualized intervention plans and comprehensive discharge plans that address the specific needs identified during assessments.

• Documenting patient interactions and clinical notes accurately and responsively to maintain up-to-date records that comply with medical standards and regulations.

• Providing therapeutic support to help patients achieve the goals identified in care plans, ensuring that interventions align with best practices for social work.

Qualifications\n

The ideal candidate for this position will possess the following qualifications:
