Registered Nurse Case Manager

Be Part Of A Growing Industry That Changes LivesInspired by nurses, Boost Home Healthcare makes it easier for patients to focus on recovery and wellness by personalizing and coordinating care.Benefits:Competitive salaryFlexible schedulePaid time offHealth insuranceWhy join our team:Treated with respect and dignityOngoing training and development opportunitiesSupported in the fieldFlexible schedulingOpportunities for extra shiftsCompetitive payWhat you'll be doing:Make the initial evaluation visit and regularly reevaluates the patient's nursing needs.Meet Daily Productivity Standards.Initiate the plan of care and necessary revisions.Provide those services requiring substantial specialized nursing skills.Initiate appropriate preventative and rehabilitative nursing procedures.Prepare clinical and progress notes for each patient visit and summaries of care conferences on their patients in a timely manner.What we're looking for:A passion to serve and help others live their best lives possible.Graduate of an accredited school of professional nursing.Currently licensed as a registered nurse in the state of Arizona.Minimum of 1 year of nursing experience in a home health setting.Must have current BLS Certification.Access to reliable transportation.Compensation: $34.00 - $42.00 per hour
