Key Responsibilities\n

• Conduct thorough assessments of clients to identify their needs and tailor individualized treatment plans accordingly.

• Maintain consistent communication with supervisors and interdisciplinary team members to monitor client progress and adjust care as necessary.

• Organize and provide a detailed schedule of client appointments and follow-ups, ensuring appropriate monitoring of treatment advancements.

• Engage in multidisciplinary team meetings to discuss case progress and share insights on client needs.

• Document monthly and quarterly progress notes accurately to ensure comprehensive care delivery and compliance with ethical standards.

• Serve as a liaison between clients, families, and community resources to promote effective support and interventions.

• Participate in additional training and certification efforts for staff to enhance program delivery and professional development.

Qualifications\n
