Registered Nurse - Hospice

$2545.92/week -

Job Summary:

Provides comprehensive end-of-life care to terminally ill patients within the hospice setting, ensuring compassionate support and adherence to regulatory standards. Manages patient caseloads by conducting assessments, developing care plans, and delivering skilled nursing interventions to meet physical, emotional, and psychosocial needs. Acts as the primary liaison for patients, families, and interdisciplinary team members to facilitate seamless, individualized care.

Responsibilities:

• 
  Conduct thorough initial and ongoing assessments of patients' physical, emotional, psychosocial, and spiritual needs.
  
  
• 
  Develop and update individualized care plans in collaboration with physicians and the interdisciplinary team.
  
  
• 
  Deliver skilled nursing interventions including medication management, pain and symptom control, and wound care.
  
  
• 
  Educate patients and families on disease processes, medication use, symptom expectations, and hospice philosophy.
  
  
• 
  Provide emotional support and crisis intervention to patients and families during challenging moments.
  
  
• 
  Ensure accurate documentation of patient care, maintaining compliance with Medicare, Medicaid, and federal hospice regulations.
  
  

Qualifications:

• 
  Hold a current Registered Nurse (RN) license in the state of practice.
  
  
• 
  Possess at least two years of recent hospice nursing experience.
  
  
• 
  Maintain current BLS certification from an approved provider.
  
  
• 
  Possess a valid drivers license, reliable automobile, and current auto insurance; must be at least 21 years old.
  
  
• 
  Demonstrate excellent communication and interdisciplinary collaboration skills.
  
  
• 
  Be willing to participate in on-call rotations and respond promptly to after-hours needs.
