Bilingual Community Health Navigator
Bronx Community Health Network
Bronx Community Health Network (BCHN) is hiring a Community Health Worker (CHW) to join the Population Health team. The CHW will promote health, reduce barriers to care, and support patients in navigating the healthcare system. This is a full-time, on-site role with a Monday–Friday schedule and some weekends as needed, reporting to the Asst. Director of Population Health.
