Patient Services Coordinator‑LPN The role is responsible for scheduling visits and communicating with field staff, patients, physicians, and other stakeholders to maintain proper care coordination and continuity of care. It also assists with day‑to‑day office and staff management. Responsibilities Manage schedules for all patients, including editing schedules for agents who call in sick and updating agent unavailability in the worker console. Initiate infection control forms as needed and send completed “Employee Infection Report” to the HRD for upload. Serve as backup during lunch hour and other busy times, receive calls from field staff, and assist with weekly case conferences; refer clinical questions to the Branch Director as necessary. Maintain the client hospitalization log, enter coordination notes, and distribute the electronic log to all office, field, and sales staff. Complete requests for skilled nurse/injection visits unless an aide supervisory visit is scheduled concurrently with the injection visit. Schedule all add‑ons and applicable orders, including discharge visits, OASIS collection or recert visits, TIF OASIS collection visits, and deletion of remaining schedules. Reschedule declined or missed visits (when appropriate) and process reassigned and rescheduled visits. Ensure supervisory visits are scheduled. Run all scheduling reports, such as Agent Summary Report and Missed Visits Done on Paper Report. Prepare weekly Agent Schedules, review for productivity or geographic issues, and forward for Branch Director approval before distribution to staff. Verify visit paper notes in the scheduling console as needed. Assist with internal transfer of patients between branch offices. If clinical, receive lab reports, assess for normality, fax a copy to the doctor, create a copy for the Case Manager, and route to the Medical Records Department; initiate Employee/Patient Infection Reports as necessary. If clinical, may be required to perform patient visits and/or participate in on‑call rotation. Qualifications Licensed Professional Nurse (LPN) or Licensed Vocational Nurse (LVN) licensed in the state of employment. At least 1 year of home health experience. Prior packet review or QI experience preferred. Coding certification preferred. Valid state driver’s license and automobile liability insurance. Excellent communication skills, ability to interact well with a diverse group of individuals, strong organizational skills, and ability to manage and prioritize multiple assignments. Compensation Scheduled Weekly Hours: 40 hours per week. Pay Range: $48,900 - $66,200 per year (based on geographic location and individual qualifications).
