Physical Therapist

We seek a highly skilled and experienced Registered Nurse with specialities in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, Operating Room. The Registered Nurse will be responsible for providing skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Daily tasks include patient assessments, medication administration, treatment planning, and collaborating with the interdisciplinary team. Critical to this role is strong communication and strong collaboration with patients, providers, and other members of our team. At least 2 years of experience and exceptional skill is required. Responsibilities:

• Provide skilled nursing care including patient assessments and medication administration.
• Develop and implement treatment plans in collaboration with the interdisciplinary team.
• Manage case and provide hospice care to patients with advancing age, frailty, and serious illness.
• Communicate effectively with patients, families, healthcare providers, and team members.
• Document patient care and maintain accurate medical records.
• Ensure compliance with healthcare regulations and facility policies.

• Active Registered Nurse license in the state of employment.
• Bachelor's degree in Nursing or related field.
• Minimum of two years experience in hospital, home health, or hospice care.
• Strong clinical assessment, medication administration, and treatment planning skills.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment.
• Driver's license, car insurance, and automobile for field work.

• Experience in specialized nursing areas such as ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room.
• Additional certifications related to nursing specialties.

• May require occasional on-call duties depending on the healthcare setting.
• Work environment may include hospice, critical care units, or surgical settings.

• Standard daytime shifts primarily Monday through Friday.
• Shift length may vary; typical schedules include 8-hour or 10-hour shifts.
• Occasional weekend shifts may be required based on patient care needs.
