Physical Therapist

The Physical Therapist will provide skilled physical therapy services within assigned county territories, including Richmond and surrounding areas, with possible additional travel. The role involves delivering patient care through evaluations, treatments, and follow-ups while maintaining productivity standards. The therapist will collaborate with patients and healthcare providers to promote recovery and mobility. Flexibility to work weekends and travel outside the service area is required to meet caseload and productivity expectations. Responsibilities:

• Conduct patient evaluations, re-certifications, and routine therapy visits.
• Provide therapeutic treatments and interventions to improve patient mobility and function.
• Complete documentation and reporting for each patient visit, including OASIS assessments.
• Coordinate care with interdisciplinary teams and communicate patient progress.
• Travel within assigned territories and occasionally outside the area to meet caseload demands.
• Adhere to agency and client policies regarding visit documentation and billing.
• Maintain compliance with infection control protocols, including care for COVID+ and PUI patients.

• Current Physical Therapist license in California.
• BLS certification.
• Valid driver's license, automobile insurance, and vehicle registration.
• Up-to-date immunizations and screenings: TB (annual), MMR, Varicella, Hepatitis B, TDap (within 10 years), Flu (annual), drug screen, and physical exam.
• Minimum of 1 year of relevant physical therapy experience preferred.
• Ability to document and complete OASIS assessments accurately.

• Experience in home health physical therapy.
• Familiarity with managing caseload productivity and visit documentation standards.
• Comfort working with diverse patient populations and complex cases.

• Weekend coverage may be required.
• Must be willing to travel outside assigned service areas as needed.
• Care for COVID+ and PUI patients is required.
• Strict adherence to submitted Requested Time Off (RTO) policies; unapproved or late RTO requests may lead to removal from vendor set.
• Productivity standards require completion of at least 27 points per week to meet guaranteed hours.

• Work schedule includes a 36-hour weekly guarantee based on productivity or per-visit payment structure.
• Shifts and assignments are determined by patient visit needs and territory coverage.
• Flexibility to work weekends and adjust schedules to maintain required visit points is expected.
