Pay Range:

$47.50 - $71.25

Join our team-oriented group of compassionate clinicians in North Charlotte/ Cabarrus area, where supportive leadership, strong work-life balance, and the opportunity to deliver exceptional patient care come together.

Position Highlights
• Monday–Friday | 8:00 AM–5:00 PM
• Shared call rotation
• In-person and virtual visits supported by Medical Assistant
• Manage complex patients with ongoing follow-up throughout stay
• Collaborate with SNF team to facilitate care coordination
• Implement discharge planning to improve patient outcomes
• Signing bonus, CME, and relocation available.

Candidate Qualifications

Ideal Candidate
• Physician Assistant or Nurse Practitioner
• Passion for working in a SNF/Post-Acute Care Environment
• Experience in a Skilled Nursing Facility is preferred but not required
