Key Responsibilities

• Conduct thorough patient evaluations to determine needs and establish effective treatment plans tailored to each individual's goals and abilities.
• Provide expert consultation to healthcare providers regarding each patient's continuum of care, ensuring seamless transitions and optimal patient outcomes.
• Maintain accurate documentation of evaluations, treatment plans, progress notes, and discharge summaries, adhering to professional guidelines and regulatory standards.
• Effectively communicate with patients, families, and interdisciplinary team members, fostering a collaborative approach to treatment and recovery.

Qualifications

Compensation and Benefits
