Work Environment

Operating within the home care setting, our therapists engage directly with patients in their residences, allowing for personalized care tailored to their unique environments. Key Responsibilities

• Conduct comprehensive evaluations and determine personalized treatment plans that align with established standards of care.
• Select and utilize appropriate assessment tools, tests, and measures to effectively evaluate the functional capabilities of each patient.
• Develop and implement targeted treatment programs designed to restore function, improve mobility, relieve pain, and prevent or limit permanent disabilities.
• Document all patient evaluations, treatment sessions, and progress thoroughly using our electronic medical record (EMR) system while ensuring accurate billing information is recorded.
• Facilitate screenings for conditions such as lymphedema and fall risks, ensuring timely interventions are provided.
• Collaborate closely with a multidisciplinary team, including physicians and other healthcare providers, to discuss patient needs and coordinate care.
• Engage in departmental education efforts, including presentations and supervision of students or interns, contributing to your professional advancement within a clinical career ladder.

Patient Population / Client Focus

Our diverse client base includes individuals with a range of physical limitations, cognitive impairments, psychosocial dysfunctions, mental health issues, and developmental disabilities. As a Physical Therapist, you will play a crucial role in delivering tailored interventions that foster independence in daily living for these patients. The ideal candidate will demonstrate compassion and adaptability in working with varied age groups, including pediatric patients, to meet their evolving healthcare requirements.

Qualifications

• Active Florida PT license.
• A bachelor's degree (or higher) in Physical Therapy from an accredited institution.
• At least one year of clinical experience in an acute care hospital setting is required.
• Strong proficiency in trigger point therapy techniques, along with a solid understanding of anatomy and physiology.
• Experience in providing care for diverse patient populations, including pediatric patients.
• Familiarity with hospital medicine practices is a plus.
• A proven ability to thrive in both clinical and home care environments.

Preferred Experience

• Master's or Doctorate degree in Physical Therapy.
• Previous involvement in educational roles, such as mentoring students or interns.
• Experience in outpatient rehabilitation services and a strong understanding of evidence-based practices in physical therapy.
