Key Responsibilities\n

• Rapidly establish rapport with patients and their families, employing active listening techniques to foster trust and effective communication tailored to individual needs and preferences.

• Collaborate with a multidisciplinary team to provide comprehensive care, ensuring all patient interactions are documented in the electronic medical record within 48 hours to maintain continuity of care.

• Conduct screenings and assessments for mental health concerns, such as depression, anxiety, and substance abuse, to identify treatment needs effectively. Facilitate referrals to appropriate professionals when necessary.

• Maintain an efficient patient caseload by providing brief interventions and follow-up support, ensuring patients receive appropriate care based on their mental health needs.

• Develop and implement integrated care plans, coordinating with community resources and other service providers to meet each patient’s unique requirements.
\n
• Participate actively in supervision sessions, case conferences, and quality improvement initiatives to continually enhance service delivery.

• Provide referrals for psychiatric evaluations and ongoing support to patients requiring medication management, in collaboration with our consulting psychiatrist.
\n
• Proactively identify community resources to assist patients in achieving their personal goals, with a focus on social, educational, spiritual, and vocational support.
\n
• Safeguard patient confidentiality and privacy in all communications, upholding the highest ethical standards in documentation and data handling.
\n
• Carry out other related responsibilities as assigned to ensure comprehensive patient care and service delivery.

Patient Population / Client Focus\n

This role focuses on an underserved patient population, particularly those facing social determinants of health that may impede their access to care. You will work with individuals who may be experiencing economic hardship, housing instability, or health inequity, providing them with the resources and care they need to thrive.

Qualifications\n

• A Master’s degree in a related Behavioral Health field is required, along with a valid LCSW license.

• CPR or BLS certification is necessary to ensure patient safety during interventions.

• Three to five years of relevant experience in the behavioral health field is mandatory, showcasing an ability to manage varied responsibilities effectively.

Compensation and Benefits\n
