• Provide counseling and supportive services to residents, families, and caregivers to promote emotional well-being and social support

• Facilitate and participate in interdisciplinary team meetings to ensure seamless coordination of care

• Develop and maintain positive relationships with residents, families, and staff to promote a culture of respect, empathy, and compassion

• Maintain accurate and timely documentation of resident assessments, care plans, and progress
