Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to gain insights into clients' mental health concerns, strengths, and resources.

• Deliver one-on-one, family, and group therapy sessions using evidence-based modalities, including cognitive-behavioral therapy (CBT) and trauma-focused therapy.
\n
• Develop and implement individualized treatment plans tailored to each client’s unique needs, including defining actionable goals and appropriate therapeutic interventions.
\n
• Collaborate closely with a diverse treatment team, including psychiatrists, psychologists, and other healthcare professionals to provide coordinated and holistic care.
\n
• Offer crisis intervention, comprehensive risk assessments, and develop safety plans as necessary, ensuring clients feel supported in their journey.
\n
• Maintain organized and timely documentation of client interactions in accordance with ethical and legal standards, leveraging electronic health record systems.
\n
• Facilitate therapeutic group sessions, psychoeducation classes, and support groups to enhance client engagement and peer support.

You will primarily work with hospice clients, which includes assisting them and their families in navigating mental health challenges during end-of-life care. This population often experiences a range of emotional responses, and your support will be crucial in fostering resilience and peace. Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Connecticut.

• Exceptional interpersonal skills with the ability to empathize and connect with clients from diverse backgrounds.

• A minimum of two years’ experience in a mental health or outpatient care setting, particularly with hospice or palliative care clients.
