Great Livin' LLC is seeking a full time professional registered RN. This position provides nursing services to adults with intellectual and developmental disabilities who are residents of GLLLC Group of Companies' Home and Community Based Waiver Program and ICF-IID. Nursing services are implemented under established GLLLC policies, and within the scope and limitations set forth in Medicaid's Conditions of Participation as related to Nursing Services through the New Mexico Developmental Disabilities Waiver and New Mexico MIA Via Waiver Medicaid Home and Community Based Waiver Standards, ICF-IID Medicaid Standards, and New Mexico Board of Pharmacy regulations. The supervision of services will be provided by GLLLC's Director or Designee as required by standards and GLLLC Subcontractor policy and procedure to oversee the services provided by the nurse. Roles and Responsibilities Complete initial nursing assessments and discharge summaries for DD Waiver and ICF-IID. Complete monthly/quarterly/semi-annual nursing reports to HCC and IDT, distributing via scomm to GL admin and IDT members, and attaching to appointment in Therap. Create and submit HCP, MERPS, CARMP annually, following hospital admission and any change in condition via s-comm and Therap per state standards. Initiate and complete nursing portion of CARMP. Train and delegate HCC on all individual’s HCP/MERPs and CARMPs. Review and sign all appointment consultation forms, communicating all follow‑ups to HCC. Monthly review and oversight of MARS: review, update, and communicate with HCC all follow‑ups with physicians’ offices and pharmacy. Complete medication review form. Complete and update Therap documentation as appropriate for individuals receiving services, including E‑CHAT/E‑CHAT summary; MAAT; ARST; current ICD/DSM codes for medications and diagnoses. Review in Therap and communicate to HCC all medication changes, appointment needs, health tracking, and vitals needed. Document medical concerns in T‑logs or ISP data. Provide follow‑up regarding GERs that involve individual’s health and medical issues, including use of ER, urgent care, and/or EMT services; injuries; medication errors. Attend individual’s / Great Livin meetings as needed to address health and medical needs, including ISP/IDT meetings; management team meetings; basic assurances; HRC; doctor appointments, audits, etc. Correspond with teams, medical providers and/or other agencies to address individual’s health and medical issues/concerns. Participate in pharmacy reviews for services provided by Great Livin as needed. Implement training and oversight of CMAs through GL’s CMA program and supervise after receiving CMA certificate. Complete all duties within timelines established by NM DD Waiver, NMAC codes, and ICF/IDD standards. Initial Employee Injury Assessment When appropriate, be available to assess employee injuries. Knowledge and Skills (present or attainable) Must be able to speak, read, and write in English. Proficient written skills and ability to use computer programs used in Great Livin's health services management. Knowledge of agency positions and responsibilities, and whom to go to with specific questions and/or concerns. Good integrity and adequate physical, mental, and emotional stability to meet job duties safely and responsibly. General understanding of the New Mexico ICF‑IDD standards and working knowledge of Board of Nursing regulations and requirements related to Great Livin's CMA education program and CMA services. Ability to establish priorities, work independently, exhibit good leadership skills, and proceed with objectives without hands‑on supervision. Ability to handle and resolve recurring problems. Willing to work the hours necessary to meet the needs of the individuals with whom you work. Valid auto liability insurance. Professional liability insurance with limits of $1,000,000 (each claim) and $3,000,000 aggregate. Pass required background checks. New Mexico professional license. Job related certification(s)/trainings, as and when deemed necessary by Great Livin' LLC and/or the state of New Mexico.
