Key Responsibilities:
- Collect and analyze relevant health data to determine diagnoses and care issues.
- Develop personalized care plans with specific interventions to achieve desired outcomes.
- Implement care plans and coordinate health delivery to promote patient safety and wellbeing.
- Evaluate patient progress and adjust plans as necessary.
- Collaborate with patients, families, and healthcare providers to foster a humane and healing environment.
- Provide educational opportunities for patients and their families, ensuring they are informed about their care.
- Participate in discharge planning to ensure continuity of care.
- Effectively delegate tasks and coordinate duties among the healthcare team.
- Perform additional job-related duties as assigned.

Qualifications:
- Bachelor's Degree in Nursing (BSN) preferred.
- At least two years of recent experience in an acute care facility preferred.
- Current Registered Nurse (RN) licensure in the state of practice, required.
- Current Basic Life Support (BLS) certification, required.
- AHA-NRP or ARC-NALS certification, required.
- S.T.A.B.L.E Certification, required.
- EPIC experience required.
