Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct detailed OASIS assessments and design individualized care plans for each patient.
• Deliver skilled nursing services, such as wound care, medication administration, IV therapy, and chronic disease management in a field-based setting.
• Monitor and evaluate patient status, reporting any significant changes to the care team and physicians for timely interventions.
• Provide education to patients and families regarding disease processes, medications, preventative strategies, and treatment plans.
• Ensure meticulous documentation of all care activities using Home Care Home Base (HCHB).
• Work in partnership with physical therapists, occupational therapists, social workers, and home health aides to ensure the highest standard of coordinated patient care.
• Attend case conferences, participate in staff meetings, and complete continuing education as required.
• Consistently adhere to all agency and regulatory requirements governing documentation and patient care.

Qualifications:

• Active, unencumbered RN license in the state of practice
• At least 1 year of recent Home Health experience (field experience highly preferred)
• Proficient with Home Care Home Base (HCHB) and OASIS assessments
• Reliable transportation, valid drivers license, and current auto insurance

$2,057 - $2,176/WeekEstimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
