Washington, DC Pay From: $22 Per Hour

MUST: 2+ years of experienced as an administrative support candidate in a community health, behavioral health, Urgent Care, Emergency Room or in patient unit experience Scheduling appointments Handling care coordination Creating Excel spread sheets to track patient information Working with clinical staff to support patients Assisting patients with transportation and community resources EMR documentation Experience working with underserved population, substance use disorders and patients with mental illness Recent acute care experience in medical Emergency, Urgent care, or other inpatient units Experience in or passion for public health and community nursing Excellent communication and interpersonal skills Experienced with Microsoft Office Suite Bachelor's Degree

DUTIES: Handle Care Coordination Provide direct support to the healthcare team Manage referrals, tracking of outside specialists, transportation to/from outside appointments. Act as a program liaison with Social, Housing and Behavioral Health providers who are working with patients admitted to the program Create and maintain Excel Spread Sheets for tracking purposes Supports patients to advocate for themselves in their interactions with these other community providers and stays abreast of efforts to house or transition patients to other long-term placements Track patient's immunizations, screenings and facilitate referrals and appointments Assist patients with financial/insurance options, transportation and translation services Assist patients with navigating the healthcare system including but not limited to working with pharmacies, social service agencies, and insurance agencies Communicate patient-related needs to appropriate clinical staff Performs other duties as assigned Quadrant is an affirmative action/equal opportunity employer.
