Admissions RN - Home Health (PRN/Flex, Weekend Coverage)

Gentiva

Heartland Home Health, part of Gentiva, is seeking a PRN/FLEX Admissions RN to welcome patients into our home health program with a smooth, professional and compassionate admission experience from day one.

You will conduct assessments, initiate care plans, and act as a liaison among referral sources, patients, families, and the care team, while ensuring compliance and high-quality patient care in home settings.
