Key Responsibilities

• Conduct comprehensive patient assessments to determine mobility limitations and specific therapy needs.

• Provide skilled physical therapy interventions directly to patients in their homes, using appropriate therapeutic techniques and modalities.
• Monitor and document patient progress accurately through electronic medical records (EMR) systems like WebPT, ensuring compliance with healthcare regulations.
• Educate patients and their families on exercises, therapeutic equipment, and strategies to integrate therapy into their daily routines for optimal recovery.
• Prioritize caseload effectively, coordinating patient assignments and ensuring seamless transitions of care for all patients.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.

Compensation and Benefits
