Work Environment

This position is onsite, engaging with clients in their homes across South River, NJ. Your office will essentially be the diverse environments of your clients' residences, which provides a unique opportunity to assess and adapt therapeutic techniques based on real-world conditions. You will work closely with an interdisciplinary team to ensure comprehensive care delivery, while fostering strong relationships with clients and their families.

Key Responsibilities

• Conduct thorough evaluations to assess clients' physical abilities and limitations, utilizing diagnostic tests designed to measure muscle strength, joint range of motion, and functional capabilities.
• Develop individualized treatment plans that align with physician prescriptions and incorporate patient goals, emphasizing the prevention of disability and enhancement of functional independence.
• Deliver physical therapy services directly in the home, administering therapeutic exercises, manual techniques, and physical agents as needed to alleviate pain and promote recovery.
• Oversee the work of Physical Therapy Assistants by providing guidance, ensuring compliance with the established care plan, and conducting regular assessments to track progress.
• Evaluate home environments for safety and rehabilitation potential, making recommendations that empower clients to achieve maximum independence.
• Document all clinical interactions in compliance with regulatory standards, including the preparation of initial evaluations, progress notes, and discharge summaries.
• Communicate regularly with clients, families, and the interdisciplinary team to discuss treatment progress, reassess goals, and modify plans as necessary.
• Participate in case conferences to contribute to the comprehensive treatment strategy for each client, offering insights that impact overall patient care.
• Instruct clients and their families on the use and care of therapeutic devices, ensuring a clear understanding of home exercise programs and rehabilitation activities.

Patient Population / Client Focus

Your primary focus will be on orthopedic conditions, commonly supporting individuals recovering from surgery, injury, or chronic pain disorders. You will be entrusted to guide these clients through their rehabilitation journey, adapting your approach to fit their specific needs and capabilities.

Qualifications

• Doctorate in Physical Therapy from an accredited program.
• Active physical therapy licensure in New Jersey.
• Current CPR certification and a valid driver's license with appropriate automobile insurance.
• A strong commitment to clinical excellence, patient advocacy, and ongoing professional development.
• Exceptional communication skills, with a focus on building rapport with clients and families.
• In-depth knowledge of federal and state guidelines governing physical therapy practices.

Preferred Experience

• A minimum of one year of experience working as a physical therapist in an acute care or home care setting.
• Experience in evaluating and administering treatment for orthopedic patients.
• Demonstrated ability to work collaboratively within a multidisciplinary team.
• Proficiency in utilizing electronic health record (EHR) systems for documentation and reporting.

Compensation and Benefits

This full-time position offers a competitive annual salary range of USD 100,000 to USD 120,000, commensurate with experience and qualifications.
