Service Coordinator - Housing Focused (Bilingual)

The Mission Neighborhood Health Center (MNHC) is a non-profit, multi-site community health organization offering comprehensive health services in the Mission and Excelsior Districts of San Francisco.  The center offers primary health care services, adult medicine, pediatrics, family planning, OB/GYN, HIV and homeless services. We are currently looking for qualified candidates for our  Service Coordinator - Housing Focused (Bilingual)role.  Please note: This role requires applicants to be bilingual in English and Spanish. This position reports to the Program manager, integrated services and is a Full-Time, Non-Exempt role with a hourly range of $24.47 to $26.28

PRIMARY OBJECTIVE

The Service Coordinator – Housing focused provides one-on-one support to individuals experiencing or at risk of homelessness, assisting clients with housing applications, individualized housing plans, and documentation. This role coordinates with housing providers and partner agencies to facilitate successful placements while maintaining accurate records in the HSH ONE System and delivering trauma-informed, culturally responsive services.

ESSENTIAL FUNCTIONS & RESPONSIBILITIES

• Provide one-on-one housing navigation services to assigned clients experiencing or at risk of homelessness, including developing individualized housing plans and identifying appropriate housing opportunities.
• Assist clients in completing housing applications and gathering required documentation, including identification, income verification, benefits, and other eligibility documents.
• Maintain regular communication with clients throughout the housing placement process, providing updates, encouragement, and promptly communicating changes, concerns, or barriers to the supervisor or case management team.
• Coordinate with property managers, leasing agents, housing providers, and partner agencies to support successful housing placements.
• Maintain accurate, timely, and confidential client records in the HSH ONE System and other required databases in accordance with HIPAA, agency, and regulatory standards.
• Provide professional, trauma-informed, culturally responsive, and linguistically appropriate services, including support for Spanish-speaking clients.
• Educate clients about available housing resources, benefits, and community services, advocate for clients, and provide referrals to internal MNHC programs and external community resources.
• Participate in outreach, case conferencing, multidisciplinary team meetings, and care coordination activities as assigned.
• Participate in required training, staff meetings, and professional development activities.
• Perform other duties as assigned to support program operations and client success.

MINIMUM QUALIFICATIONS

• High school diploma or equivalent.
• Bilingual in English and Spanish.
• Two years of experience providing case management, housing navigation, intake and assessment, service planning, documentation, referrals, or other direct services to individuals experiencing homelessness or housing instability.
• Proficiency in Microsoft Office Suite (Word, Outlook, Excel) and the ability to quickly learn electronic case management systems, including ONE System, Epic, or similar databases.

PREFERRED QUALIFICATIONS

• Associate degree in Human Services, Social Work, Psychology, Behavioral Health, or a related field.
• Completion of a Case Management Certificate or other relevant workforce training program.
• Experience using electronic databases such as ONE System, Epic, or similar case management systems.
• Knowledge of San Francisco's homelessness response system, Adult Coordinated Entry, and housing resources.
• Experience working in homeless services settings such as shelters, navigation centers, resource centers, HSH-funded programs, permanent supportive housing, rapid rehousing, behavioral health programs, or community-based organizations.
• Experience working with individuals experiencing homelessness, serious mental illness, substance use disorders, or other vulnerable populations; lived experience welcomed and valued.
