You will play a critical role in the Hospice Center's treatment process, collaborating closely with a dedicated team of healthcare professionals, including doctors, nurses, and fellow social workers. Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for assigned patients, identifying behavioral health needs, strengths, and support networks.

• Develop and implement tailored clinical interventions, including individual, group, and family therapy, to support patients facing terminal illness and their families.

• Coordinate care by working with the interdisciplinary treatment team to make informed decisions regarding patient treatment plans.

• Respond to and provide crisis intervention during urgent situations, such as mental health emergencies or child abuse cases.
\n
• Research and advocate for community resources, including healthcare, financial support, and social services, to improve client well-being.
\n
• Engage in regular follow-ups to monitor client progress and make adjustments to care plans as needed, continuously aiming for the best patient outcomes.

• Document all assessments, interventions, and progress notes following ethical and legal guidelines, utilizing electronic health record (EHR) systems effectively.

Patient Population / Client Focus\n

The primary focus of this role is on patients receiving hospice services, encompassing individuals with terminal illnesses and their families. You will support clients through sensitive transitions, providing emotional support, grief counseling, and connection to vital resources that can enhance their quality of life during this challenging time.

Qualifications\n

• A Master's Degree in Social Work from an accredited institution.

• Must hold a California BBS Licensed Clinical Social Worker (LCSW) credential.

• A minimum of 2 years of experience in a behavioral health setting, specifically working with patients in hospice or palliative care environments.

• Proficient in conducting psychosocial histories and evaluations, including crisis assessments and case management.

• Strong skills in individual, group, and family therapy modalities.

• Basic understanding of community resources and the ability to connect patients to essential services.

Compensation and Benefits\n
