Key Responsibilities\n

• Engage directly with clients through virtual consultations, assessing and addressing symptoms related to major mental illness and severe emotional disorders.

• Collaborate with a multidisciplinary team to establish and achieve shared treatment goals, ensuring that each client receives holistic and individualized care.
\n
• Conduct evidence-based screenings and assessments, developing integrated care plans that link clients to necessary services and support.
\n
• Provide crisis intervention support, employing therapeutic techniques to stabilize clients and connect them with additional resources as needed.

• Monitor and facilitate group therapy sessions, individual counseling, and family therapy, focusing on the client’s presenting problems and their personal treatment goals.

• Participate in continuous quality improvement initiatives, attending regular meetings and contributing to the enhancement of agency service delivery.

• Ensure the timely completion of documentation, reporting, and data systems to maintain compliance with internal and external standards.

• Provide on-call support during assigned after-hours periods to ensure ongoing client care and crisis management.

Patient Population / Client Focus\n

The LCSW will work primarily with clients who are experiencing severe emotional distress and mental illnesses, often in a hospice context. This involves addressing complex psychological needs while supporting these individuals through their end-of-life concerns. Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Current LCSW licensure in the state of Connecticut.

• Exceptional verbal and written communication skills, facilitating effective collaboration with clients, families, and care teams.

• Strong organizational skills, capable of managing multiple caseloads and documentation requirements efficiently.

Preferred Experience\n

• A minimum of 2 years of clinical experience in social work, preferably within a mental health setting focused on end-of-life care or hospice environments.

Compensation and Benefits\n

The salary for this position ranges from USD 70,000 to 85,000 annually, commensurate with experience and skills.
