Key Responsibilities\n

• Conduct comprehensive assessments of patients' needs, behaviors, and emotional responses using interviews and direct observations.

• Design and implement tailored treatment plans that address mental health issues such as anxiety, depression, terminal illness grief, and coping with chronic/acute pain.

• Facilitate psychotherapy groups aimed at empowering patients to share experiences, learn coping mechanisms, and build community support.

• Collaborate with physicians and other healthcare professionals to provide a multifaceted approach to mental health and well-being.

• Maintain accurate and timely documentation of patient interactions and treatment plans in compliance with federal and state regulations.

Patient Population / Client Focus\n

In this role, you will primarily work with patients facing psychiatric illnesses, including mood disorders, anxiety disorders, thought disorders, and personality disorders. You will be instrumental in providing care to individuals in hospice, helping them navigate the psychological challenges associated with end-of-life issues. Your expertise will also support family members as they cope with their loved ones' conditions, ensuring a comprehensive approach to mental and emotional support.

Qualifications\n

• A Master’s Degree in Social Work from an accredited program.
