Physical Therapist (Home Health)

Pride Health is hiring a Physical Therapist (Home Health) to support our client's healthcare facility in Winter Haven, FL.

Job Details:
Schedule: Monday-Friday | 8:00 AM-4:30 PM
Location: Winter Haven, FL
Coverage Area: Polk County
Contract Length: 10+ Week Contract (Possibility to Extend)
Job Type: Local contract [Travel contracts are also available if you reside more than 50 miles from the specified location]
Pay Range: $52.00 - $56.00/hour (W-2) Plus Mileage reimbursement
Mileage Reimbursement Rate: $0.49 mile
*Pay offered is based on experience, expertise, credentialing, and education

Duties:

• Perform comprehensive physical therapy evaluations and ongoing reassessments to identify impairments, functional limitations, and safety risks.
• Develop, implement, and adjust individualized care plans to improve mobility, strength, balance, endurance, pain management, and functional independence.
• Deliver skilled interventions including therapeutic exercise, gait training, transfer training, balance activities, stair training, and home safety education.
• Educate patients and caregivers on home exercise programs, fall prevention, assistive device use, and energy conservation techniques.
• Complete accurate and timely documentation (evaluations, visit notes, progress reports, and discharge summaries) using EMR systems.
• Coordinate care with physicians and interdisciplinary team members, including nurses, occupational therapists, speech therapists, and home health aides.
• Communicate changes in patient condition, safety concerns, or barriers to progress promptly to the care team.
• Participate in case conferences, care coordination activities, and team collaboration.
• Ensure compliance with CMS regulations, infection control standards, and agency policies.
• Manage a mobile caseload efficiently while meeting productivity and documentation expectations.
• Adapt quickly to new environments, workflows, and patient populations as a travel clinician.

Requirements:

• Education: Master's Degree
• License(s): Active Physical Therapist license
• Certification(s): Current CPR certification
• Years of experience: At least 1 year of Geriatric home health experience and experience with HomeCare HomeBase (HCHB) is a plus
