Case Manager

The Case Manager is responsible for providing case management services to individuals and families experiencing homelessness or housing instability. This position supports households throughout the intake, eligibility verification, housing navigation, unit placement, and stabilization, and accessing wrap around services process to ensure timely access to safe, habitable housing and supportive services. The Case Manager serves as a primary point of contact for assigned households and works closely with landlords, property managers, service providers, program administrators, and internal staff to move people from homelessness to stable housing. The ideal candidate is compassionate, organized, detail-oriented, and comfortable working with people experiencing crisis, trauma, mental health challenges, substance use disorders, or other barriers to housing stability. This position requires strong case management, documentation, communication, and problem-solving skills, as well as the ability to manage a high-volume caseload while maintaining compliance with program requirements and using case management software. Employment Type Full-Time, Temporary/Term Position (Approximately 6-12 Months) 40 hours per week Essential Duties and Responsibilities Intake and Enrollment Conduct intake assessments for individuals and families seeking housing assistance Screen households for program eligibility according to funding source requirements, including income, homelessness status, vulnerability, and household composition Enroll eligible households into housing assistance and supportive service programs Explain program rules, participant responsibilities, and documentation requirements Build rapport and establish trust with people experiencing homelessness or housing instability Eligibility Verification and Documentation Collect, review, and verify required documentation, including but not limited to: Identification documents, Social Security cards, Birth certificates, Income verification, Disability verification, Homelessness verification, Lease documents Ensure all required documents are complete, accurate, and timely Maintain organized electronic and physical files Follow all confidentiality and data privacy requirements Housing Navigation and Placement Assist households in identifying appropriate housing units based on program requirements and household needs Coordinate communication between households, landlords, property managers, and housing partners Support households through application, approval, lease-up, and move-in processes Help resolve barriers to housing placement Unit Inspections and Habitability Conduct or coordinate unit inspections prior to move-in Confirm units meet habitability, safety, and program requirements Document inspection findings and deficiencies Coordinate corrective actions when units fail inspection Ensure housing placements comply with applicable local, state, and program standards Housing- focused Case Management Maintain ongoing communication with assigned households Conduct regular check-ins and case conferencing Connect households to supportive services such as employment assistance, mental health services, healthcare, childcare, transportation, financial counseling Monitor progress toward housing stability and self-sufficiency Travel to meet clients in-person Data Entry and System Management Enter and maintain accurate records in the Homeless Management Information System (HMIS) Update all case notes, milestones, and program status changes in real time or within required timelines Track enrollments, exits, housing placements, and service delivery metrics Ensure data quality, completeness, and compliance with agency standards Reporting and Compliance Prepare regular case status reports for supervisors and program leadership Provide routine updates on caseload progress Monitor deadlines and ensure timely completion of program milestones Support audits, file reviews, and compliance monitoring Ensure adherence to agency policies, funder requirements, and standard operating procedures Training and Professional Development Attend required in-person onboarding and ongoing training on program policies and procedures, housing navigation workflows, HMIS usage, trauma-informed care, fair housing, and confidentiality and data security Participate in staff meetings and case review sessions, as required Manage an active caseload of approximately 25 households at one time Balance urgent housing placements with ongoing participant support and documentation requirements Prioritize cases based on urgency, vulnerability, and program deadlines Required Qualifications Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, Public Administration, or related field preferred OR Equivalent combination of education and relevant experience Minimum 1 year experience in: case management, homeless services, housing navigation, social services or human services program administration Preferred Qualifications Experience working with homeless response systems, shelters, housing authorities, or nonprofit service providers Experience with Continuums of Care (CoCs) or similar agencies, supportive housing providers, or homelessness prevention programs Experience using HMIS or comparable case management systems Knowledge of Rapid Rehousing (RRH), Permanent Supportive Housing (PSH), homelessness prevention programs, HUD-funded programs, Coordinated Entry systems Required Skills and Competencies Strong interpersonal and relationship-building skills Ability to work effectively with vulnerable populations in crisis Trauma-informed and conflict-resolution skills Ability to maintain confidentiality and professionalism Proficiency with Microsoft Office Suite, Cloud-based databases, HMIS or similar systems, Electronic document management systems Physical Requirements Must be based in the Dallas/Fort Worth area Must have reliable transportation and auto insurance Ability to sit, stand, walk, and travel locally for trainings, meetings with supervisors and clients, and complete housing visits or inspections Ability to transport files, laptops, or materials as needed Ability to conduct field visits to shelters, housing units, and community partner locations This is a mobile role focused on community-based case management services and daily travel throughout Dallas and Collin Counties . Work settings will include office settings, and may include participant homes, shelters, housing units, and community partner sites. The role may also involve working with people experiencing crisis, trauma, mental illness, or other complex housing barriers. Compensation Hourly Rate: Up to $25.00 per hour, commensurate with candidate's experience and education.
