Key Responsibilities\n

As a Licensed Professional Counselor at Mountain Springs, your primary responsibilities will include:

• Conducting thorough patient intake assessments to evaluate mental health and psychosocial needs.

• Collaborating with primary care providers to devise and implement tailored treatment plans that address individual patient goals.
\n
• Providing brief therapeutic interventions for patients requiring mental health support, ensuring techniques are grounded in evidence-based practices.
\n
• Monitoring patient progress and adjusting treatment strategies as necessary, including the development of relapse prevention plans where indicated.
\n
• Identifying at-risk patients and devising plans to prevent deterioration of their psychological or emotional condition.
\n
• Documenting all patient interactions and treatment outcomes clearly and accurately in compliance with clinical guidelines.

• Coordinating care with external treatment providers, as appropriate, to ensure continuity of care for patients.

• Offering education and support to patients to enhance their self-management skills and engage actively in their recovery journey.

• Contributing to program evaluation and improvement initiatives aimed at enhancing behavioral health services within the clinic.

Qualifications\n

To be considered for this role, candidates must possess the following qualifications:
