Position includes working with clients who participate in specialized programs such as LGBTQ+ and older adult populations. Provide community-based case management services such as but not limited to, symptom monitoring, treatment/service planning, assistance in daily living, supportive services, crisis intervention, linkage/referral/advocacy to community support systems and support of client’s self-determination. Work in a supportive team environment as well as collaboration with a multi-disciplinary clinical treatment team. What We Value in our next ADULT CASE MANAGER High School Diploma or Equivalent, bachelor’s degree is Preferred. Case Management experience is not required. Transportation and good driving record required as well as personal vehicle insurance for bodily injury/property damage liability. Strong interpersonal skills and ability to provide support in person and by telehealth. Must be able to lift/push/pull up to 20 pounds to support demands within office environment.
