Key Responsibilities\n

• Perform thorough and age-appropriate bio-psychosocial assessments to understand the mental health, substance use, and medical needs of clients during intake sessions.

• Provide individual, couples, and family counseling services, developing tailored treatment plans based on comprehensive evaluations.

• Document all client interactions meticulously in electronic health record systems, including progress notes, treatment interventions, and crisis situations.

• Conduct safety assessments and risk evaluations, with a particular focus on suicide risk; escalate urgent needs to appropriate mental health professionals as necessary.

• Facilitate group therapy sessions, employing evidence-based practices and engaging tools to foster connection and healing among participants.

• Collaborate with local agencies and community resources to coordinate care and provide referral services to clients, ensuring seamless access to medication assistance programs and other vital support.

• Work in conjunction with Quality Improvement teams to collect and analyze relevant data, contributing to the enhancement of our service delivery model.

Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

Compensation and Benefits\n
