Key Responsibilities\n

• Conduct age and culturally appropriate bio-psychosocial assessments to identify mental health, substance abuse, and medical needs of patients in the hospice setting.

• Offer behavioral health services to individuals, couples, and families, including counseling sessions, intake assessments, diagnosis, treatment planning, crisis intervention, and referrals to substance abuse treatment.
\n
• Document patient evaluations meticulously using electronic health records (EHR) systems, including mental health evaluations, progress notes, and suicide risk assessments, ensuring that all data is current and accurate.

• Act as a liaison between patients, families, and community resources, helping to connect them to support services, including medication assistance programs and local health agencies.
\n
• Collaborate with interdisciplinary teams, including physicians and nurses, to enhance patient care quality, monitor patient progress, and share insights on behavioral health trends.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of three (3) years of post-licensure clinical experience, specifically within a mental health, or hospice setting.

• In-depth knowledge of counseling techniques, crisis intervention, and the ability to make independent clinical judgments essential for direct service delivery.

• Strong communication skills with the ability to facilitate group sessions and contribute to community engagement programs.

Preferred Experience\n

• Previous experience working in a hospice or palliative care environment is highly desirable.

Compensation and Benefits\n
