NewYork-Presbyterian Hospital is seeking a Health Priorities Specialist – Patient Navigator (Evenings) who speaks Spanish to guide patients through the continuum of care. You will coordinate the patient experience from ER to discharge, educate patients about health issues, and connect them with community resources to support continued care. The position requires on-site presence in New York, NY, with a Tuesday–Saturday 3:00 pm–11:30 pm schedule and a strong emphasis on patient education and #J-18808-Ljbffr nyp
