Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for patients and their families, identifying emotional, social, and environmental factors that affect their treatment and life situation.

• Develop and implement individualized discharge plans by collaborating with the healthcare team and engaging patients and family members in setting realistic goals.

• Provide therapeutic interventions through various modalities, including crisis intervention, short-term and long-term individual therapy, as well as group and family therapies to enhance emotional and social coping mechanisms.

• Educate patients and families about the implications of illness, treatment options, and adjustment strategies to promote understanding and adaptation to challenging circumstances.

• Facilitate effective communication and coordination with interdisciplinary team members, including physicians and nursing staff, to optimize patient outcomes.

• Lead discharge planning activities that ensure a smooth transition from the hospice setting and establish necessary post-discharge resources, including aftercare support.

• Participate in on-call rotations to provide after-hours support and responsiveness to patient and family needs.

• Contribute expertise as a consultant within the organization and provide educational sessions on psychosocial oncology at conferences and workshops.

• Engage in community outreach initiatives that strengthen local support services for patients and families affected by cancer.

• Support the professional development of social work interns and colleagues by offering training, supervision, and mentorship, fostering knowledge advancement within the field.

This role primarily serves patients within a hospice care setting, focusing on individuals dealing with end-of-life issues, chronic illness, and cancer diagnoses. Compensation and Benefits\n

The salary for this position ranges from USD 70,000 to USD 100,000 annually, commensurate with experience and qualifications.
