RN Case Manager HospiceShalom Hospice of Washington is seeking a compassionate and experienced Registered Nurse (RN) to join our team as a Case Manager in and around Kitsap County. As a RN Case Manager, you will plan, organize, and direct hospice care for terminally ill patients and their families, providing physical, emotional, and spiritual comfort and support. If you are passionate about delivering high-quality patient care and are looking for a rewarding career opportunity, we encourage you to apply.Essential Job Functions/Responsibilities:Patient Care: Completes comprehensive assessments of patient and family to determine hospice needsProvides professional nursing care utilizing the nursing processInitiates and revises plan of care as patient status and needs changeCommunication: Maintains accurate and relevant clinical notes regarding patient's condition and care givenCommunicates with physicians, community health related persons, and families/caregiversReports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agencyAdditional Duties: Participates in on-call duties as defined by the on-call policySupervises ancillary personnel and delegates responsibilities when requiredFulfills the obligation of requested and/or accepted case assignmentsPosition Qualifications:Graduate of an accredited school of nursingRegistered Nurse, with a License in good standing to practiceMinimum of one (1) year of experience as a professional nurse within the last three (3) years or a baccalaureate degree in nursing and minimum of two (2) years' experienceExcellent observation, verbal and written communication skills, problem solving skills, basic math skills, and nursing skills per competency checklistAbility to lift, position, and/or transfer patients, and to lift supplies and equipmentPossesses and maintains CPR certificationWhat We Offer:Competitive hourly rate: $104,000- $114,400 Annual Salary with a $7,000 Sign-On Bonus available for this specific position if job started on or before September 1st, 2026.
