About Pinnacle Provider Group

Pinnacle Provider Group is a mobile medical practice serving patients throughout Southwest Florida. Our providers deliver primary care and other medical services directly to patients in their homes, assisted living facilities, memory care communities, and other residential settings.

We are growing our network of healthcare partners and are seeking an outgoing, motivated, and results-driven Healthcare Sales & Business Development Representative to help expand our presence throughout Southwest Florida.

Position Overview

The Sales & Business Development Representative will build and maintain relationships with home health agencies, assisted living facilities, memory care communities, senior living communities, rehabilitation facilities, hospice organizations, and other healthcare partners .

The primary goal is to educate healthcare organizations about Pinnacle Provider Group's services and develop referral relationships for patients who need access to a primary care provider.

What You Will Promote

Pinnacle Provider Group can assist healthcare partners with:

• Mobile primary care visits for patients who have difficulty leaving their homes or facilities
• Establishing primary care for patients who currently do not have a PCP
• Provider follow-up for home health patients
• Evaluation and management of acute and chronic medical conditions
• Signing and managing appropriate home health orders and plans of care
• Evaluating patients for skilled nursing, physical therapy, occupational therapy, and other medically necessary services
• Medication management and routine follow-up
• Laboratory and diagnostic imaging orders
• Transitional care following hospitalization
• Coordination with home health, hospice, therapy, specialists, facilities, patients, and families
• Telehealth when clinically appropriate

Responsibilities

Develop relationships with healthcare organizations throughout Southwest Florida.

Visit home health agencies, assisted living facilities, memory care communities, rehabilitation centers, senior communities, and other potential referral partners.

Introduce Pinnacle Provider Group and clearly communicate how our clinical services can support their patients and clinical teams.

Schedule meetings and presentations with administrators, Directors of Nursing, case managers, marketers, social workers, discharge planners, and other healthcare decision-makers.

Develop new referral relationships while maintaining communication with existing partners.

Represent Pinnacle Provider Group professionally at meetings, networking events, and community events.

Maintain organized records of outreach activities, contacts, meetings, referrals, and follow-up opportunities.

Preferred Qualifications

Previous experience in healthcare sales, home health marketing, hospice marketing, senior living marketing, medical sales, provider relations, or healthcare business development is strongly preferred.

Established relationships with home health agencies, assisted living facilities, senior communities, or other healthcare organizations in Southwest Florida are a plus.

The ideal candidate should have excellent communication and relationship-building skills, be comfortable conducting in-person outreach, be self-motivated, and be able to work independently.

Knowledge of the Southwest Florida healthcare market is highly desirable.

Reliable transportation is required for local outreach and facility visits.
