Work Environment

As a member of our clinic, you will be working primarily in a home care setting, which allows you to offer specialized therapy services in a comfortable and familiar environment for patients. Key Responsibilities

• Conduct thorough evaluations to review patient histories, perform specific assessments, and establish personalized treatment plans based on individual needs and goals.

• Effectively communicate evaluation results and treatment protocols to patients and their families, fostering collaborative goal-setting.

• Implement evidence-based treatment protocols while closely monitoring patient progress and adjusting plans as necessary to ensure optimal outcomes.

• Maintain open communication with other healthcare professionals to coordinate care, documenting changes in patients' conditions that necessitate alterations in treatment.

• Ensure accurate and timely documentation of patient care activities, including initial evaluations, daily treatment notes, interim progress reports, and discharge summaries.

• Adhere to established productivity standards and actively participate in departmental initiatives aimed at improving patient care and departmental efficiency.

• Engage in marketing activities both within the hospital and community to promote effective access to our services.

Patient Population / Client Focus

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.

Preferred Experience
