Work Environment

This position requires onsite engagement within a home care environment. Key Responsibilities

• Conduct comprehensive evaluations of patients, including a thorough review of medical histories, physical examinations, and functional assessments to formulate tailored treatment plans.
• Successfully communicate evaluation findings and collaboratively set achievable goals with patients and their families, ensuring their active participation in the healing process.
• Implement effective treatment protocols based on individualized assessments and adapt treatment methods to evolve with the patient's needs and the healthcare landscape.
• Maintain open lines of consultation with other healthcare providers to coordinate care, optimizing treatment outcomes and documenting any necessary adjustments to the care plan.
• Ensure proper discharge planning and transition back to home life, providing clear instructions and follow-up care options while maintaining timely and accurate documentation of patient progress and therapy outcomes.

Patient Population / Client Focus

The Physical Therapist will work primarily with patients experiencing a variety of musculoskeletal and neurological conditions, including post-operative recovery, chronic pain syndrome, and spinal injuries. Our patient-focused approach emphasizes individualized treatment and rehabilitation goals, taking into account each patient's unique circumstances and aspirations.

Qualifications

• A valid and active Physical Therapy license in the state of Florida.
• A Bachelor's Degree in Physical Therapy from an accredited institution.

• Ability to manage time effectively and prioritize patient care activities in a home care setting.

Preferred Experience

Compensation and Benefits

We offer a competitive salary in the range of USD 100,000 – 120,000 per year, commensurate with experience and qualifications.
