Key Responsibilities\n

• Conduct comprehensive assessments to identify the unique needs of each patient, tailoring treatment plans accordingly.

• Maintain regular communication with supervisors, direct care staff, and family members to discuss client progress and any required adaptations in care.

• Coordinate home visits and telehealth appointments to effectively monitor client progress and ensure adherence to treatment protocols.

• Participate in Interdisciplinary Team Meetings, contributing insights to improve patient care strategies and overall program plans.

• Document all client interactions, including progress notes and data analysis, to maintain accurate records and facilitate ongoing treatment.

• Provide training for staff and colleagues to enhance competency in implementing program plans and improving client outcomes.

• Serve as a liaison between community resources and clients to enhance their support systems and treatment pathways.

• Contribute to the development and improvement of clinical procedures and training materials aimed at enhancing service delivery.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Possession of active and unrestricted licensure as a Professional Counselor (LPC) in the state of Georgia.

• A strong understanding of legal and ethical standards in mental healthcare is essential.

Compensation and Benefits\n

The position offers a competitive salary ranging from $90,000 to $100,000 annually, commensurate with experience and caseload responsibilities.
