Empath Home Health, a part of Empath Health, is seeking a compassionate full-time Home Health PT Physical Therapist to work with us to provide individualized care in Hendry county (Clewiston, LaBelle, Lehigh Acres, and surrounding areas).

Locations: Home health visits in Hendry county (including Clewiston, LaBelle, Lehigh Acres, Moore Haven, Buckingham, and surrounding areas)

Schedule: Full-Time; M-F 40 hours with required weekend/holiday rotations as needed

What you'll do:

• Provide physical therapy services to patient as prescribed by a physician in compliance with agency policies and procedures and federal and state regulations.
• Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to determine level of functioning and patient's ability to perform activities of daily living.
• Develop and revise plans of care, in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of mobility.
• Reports to the physician and designated agency representative patient's reaction to treatment and any changes in the patient's condition.

What you'll need:

• Active Florida PT license
• Minimum of 1 year experience as a licensed Physical Therapist (PT).
• Home health care experience strongly preferred.
• Strong assessment, critical thinking, and patient education skills
