Key Responsibilities\n

• Conduct initial psychosocial assessments and ongoing evaluations to identify patient needs, employing effective interviewing techniques and social work theories.

• Hold responsibility for a diverse caseload, addressing varying degrees of clinical acuity with professionalism and care while ensuring high-quality patient outcomes.

• Provide a spectrum of therapeutic services, including supportive counseling, grief counseling, and crisis intervention, to help clients and families navigate psychological and environmental challenges resulting from illness.

• Engage in case finding, actively seeking out patients and families who may require psychosocial support, and attending interdisciplinary meetings to exchange insights and treatment strategies.

• Develop individualized treatment plans in collaboration with the healthcare team, ensuring clients receive integrated care tailored to their specific needs.

• Deliver psychoeducation to clients and families regarding diagnoses, treatment options, and adjustment strategies to assist in transitions between youth and adult services.

• Participate in quality assurance initiatives aimed at enhancing social work practices within the clinic and advocate for modifications in policies that impact service delivery.

• Assist in the clinical supervision of Master’s level social workers and interns, fostering an environment of growth and professionalism within our team.

• Ensure thorough documentation by maintaining accurate electronic medical records in compliance with departmental protocols, compiling reports as necessary, and communicating patient progress effectively.
