Enjoy the flexibility of per diem work as a Nurse Practitioner specializing in home health risk assessments. This critical role involves traveling within Clay County, IN, including Brazil, Clay City, and Knightsville, to provide in-home health risk assessments for Medicare Advantage plan members. Each assessment typically lasts around 45 minutes and may cover either annual wellness checks or post-discharge evaluations depending on the patients needs.

• Flexible shifts available between Monday and Sunday

• Minimum weekly or bi-weekly hour requirements

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments for Medicare Advantage members
• Review patients current and prior medical history, conditions, and medication usage
• Educate patients on their medical conditions and prescribed medications
• Identify and address gaps in care, offering clear recommendations for follow-up
• No requirement to create treatment plans or prescribe medicine

Preferred Qualifications:

• Active Nurse Practitioner License in Indiana
• Board Certification via AANP or ANCC
• Current CPR certification (BLS)
• MSN degree from an accredited institution
• At least 6 months of nurse practitioner experience preferred
